Author / Creator Information Submission Form

The Mormon Literature & Creative Arts database invites the submission of information about the lives and works of Mormon authors and creative personnel, as well as information about any others whose works have included Mormon subject matter. Please use this form to submit biographical details and a list of publications.

A sample entry in the database shows how information gathered will be displayed.

Data submitted on this form will be approved by a member of the Mormon Literature & Creative Arts database committee prior to being posted, and edited for length and consistency with other entries. The MLCA committee does not guarantee that any or all data submitted will be entered into the database, and reserves the right to exclude information in order to conform to the editorial and subject parameters of this resource, or to delay data entry for purposes of verification.


Fill in only those fields for which you would like information to become public. The MLDB wishes to respect the privacy of authors, and fields left blank on this form (such as the name of a spouse or children) will not appear in the database for that author. The only exception to this policy is when data about an author is a matter of public record (such as date of birth listed in a library catalog).

This form is exclusively for authors or their agents. If you wish to submit corrections, updates, or other information about a given author, please do so by sending email to the MLDB Committee at MormonLit@byu.edu.

Author Name:
Please write your name in the form used most commonly in your publications.

Other Names:
Please enter any variations on your name by which you may have been published, such as maiden names, pen names, or pseudonyms. These will appear in an “AKA” field and be searchable.

Religion:
If you wish your religious affiliation to be public you may enter it here. Please note that the Mormon Literature Database is oriented to the cultural and literary aspects of Mormonism, and no attempt is made to ascertain or verify whether people in the database have membership in the Church of Jesus Christ of Latter-day Saints. This database includes authors of many faiths who have written about Mormons and Mormonism. For works to be included in the database which are not explicitly Mormon in nature, there needs to be shown some connection between the author and the Mormon faith or culture, but this connection need not be church membership. For certain authors, religious affiliation is a matter of public record. Any religious affiliation indicated here will be annotated as “self-identified” in the database.

Enter this only if you wish this information to become public. Please note that date of birth is often a matter of public record already. For example, the MLDB draws dates of birth from library cataloging data.

If you are the literary agent for a deceased author, please indicate the author’s death date.

Many users of the database would like to know about missions served by authors in the database. If you wish, indicate the mission name(s) and the dates of service for any missions you have served.

If you wish, indicate where your schooling has been. This can either be a simple list of institutions and/or degrees earned, or a short description of your schooling.

Career:

If you wish, provide a paragraph or two describing your career. This may include brief mention of publications or activities, not simply a record of where one has worked. There are separate fields below for a complete list of publications and awards.

Example:
Career: In her professional capacity Elouise M. Bell speaks throughout the state of Utah on subjects such as poetry, journal-keeping, and feminist issues. In recent years, Bell has performed a one-woman play, Aunt Patty Remembers, throughout the Western states, which she wrote based on research in the journals of Mormon midwife and pioneer Patty Bartlett Sessions. Her publications include essays, reviews, stories, and poems in such periodicals as English Journal, Women Studies Quarterly, BYU Studies, Network, Dialogue, Sunstone, and The Ensign.

Please indicate the names of any professional groups with which you are affiliated, especially if these are relevant to literature, the arts, or Mormonism.

Please indicate any literary awards you have received, including the title of the award, the work that was honored, and the date the awards was given.

Please include any other information you would like included about yourself that might be of interest to those using the Mormon Literature Database.
